Gates are where you put them — you decide how many and where, on the Pipeline tab. Opening one is yours, in the Inbox or from your own session with an owner token. The agent token has neither the gate nor the settings — not by policy text, by the toolset.

No pass without a record.

An item shows Verified only when an independent pass wrote one. Otherwise it says so.
